Forms for Participation in EVG Athletics

All students wishing to participate in athletics at Everglades City School must have a physical, parent consent, and proof of insurance on file with the activities office. These forms must be filed before engaging in any type of conditioning, try-outs, or practices.

  1. 2012-2013 EVG Athletics Packet with signature page, parent letter, FHSAA Calendar, Rule #34 Summary of Student Code of Conduct, and checklist. Annual
  2. Random Drug Testing Consent form (must be signed by parent and student, with contact phone number) Annual
  3. EL2 FHSAA Sports Physical form (must have physician signature on this form). Annual
    • Sports Physicals: You will need Form EL2 signed by a parent, student-athlete, and then the doctor. Remember, this is an annual form required by the FHSAA. Student-athletes may not practice or play unless the sports physical form is valid. Please contact Mrs. Gilmore if you have any questions at 377-9823.
  4. EL3 FHSAA Consent and Release form (must have parent and student sign form that was revised as of 4/12). Annual
  5. EL3CH FHSAA Concussion and Heat Related Illness Consent Form (NEW FORM must be signed by parent and student). Annual
  6. ImPACT Concussion Consent form (must be signed by student and parent prior to any practice or game play). Two Years
    • ImPACT Concussion Consent form for Athletics must be signed and returned for school files. If this is not received, your child will not be testing and will not be eligible to play or practice any FHSAA sports. This includes volleyball, basketball, cheerleading, baseball, and girls' softball. If a student misses the concussion testing on the 18th, they will have to make their own arrangements to travel to Lely and meet with their athletic trainer and will NOT be able to play until this is done.

Code of Conduct

 Attention all Students who are members of Athletic Teams and other Performing and Competitive Extracurricular Activities:There is an important change to the Code of Student Conduct that you need to be aware of as we head into the summer.

Rule 34, the Activity / Athletic Code of Conduct, has new language that will go into effect IMMEDIATELY.

 “Following a participant’s first career participation in an extracurricular activity, wrongful acts that occur at any time in or out of school, 365 days a year, will be subject to code enforcement.” This means that any code violation that occurs over the summer vacation will be enforced when school resumes in August. All FHSAA member school coaches are required to view the FREE online education course “Concussion in Sports – What You Need to Know” prior to the start of practice. This NFHS concussion course may be viewed online at www. nfhslearn.com.
